Measuring for shims Hot or Cold?

Im sure their is a link here somewhere to a previous discusion on this subject, however i dont know where.
My question's are: 1) When measuring the gap for calculating the required shim, should this be carried out on a hot or cold engine? 2) and what is the exact procedure.
